This website is operated by Next Chapter with Dr. Nishi LLC (“Next Chapter,” “we,” “us,” or “our”), a life-coaching business. Our coaching services are not medical care or psychotherapy.

2. Information you choose to provide

When you use the contact form, you may choose to provide your name, email address, the type of support you are seeking, and a short message. We may also receive information you provide when scheduling a call, exchanging email, completing coaching paperwork, or purchasing services.

Contact-form submissions are processed by FormSubmit and delivered to our business email inbox. FormSubmit may temporarily process or retain submission data under its own privacy practices. Please do not submit medical records, diagnoses, treatment information, Social Security numbers, financial account details, or other highly sensitive information through this website.

3. Information collected automatically

Like most hosted websites, our hosting and security providers may automatically process limited technical information, such as IP address, browser type, device type, approximate location, requested pages, timestamps, and security logs. This information is used to deliver, protect, and troubleshoot the website.

We do not currently use advertising trackers, behavioral advertising, or a site analytics service. If that changes, this policy will be updated before those tools are enabled.

6. Coaching information and HIPAA

Next Chapter provides life coaching, not health care. The website and ordinary coaching communications are not intended to be a patient portal or a system for transmitting protected health information. Do not assume that information sent through this website is protected by HIPAA. Whether HIPAA applies depends on the legal status and activities of the parties involved; this policy does not promise HIPAA coverage.

7. Retention and security

We retain personal information only as long as reasonably needed for the purposes described above, to provide services, maintain required records, resolve disputes, and meet legal obligations. We use reasonable administrative and technical safeguards, but no electronic transmission or storage method can be guaranteed completely secure.

You may request access to, correction of, or deletion of personal information we maintain about you, subject to legal and recordkeeping exceptions. You may also ask us not to send nonessential communications. Additional rights may apply depending on where you live.

This website and our coaching services are intended for adults and are not directed to children under 18. We do not knowingly collect personal information from children through this website.
